Background & Compliance Requirements

7-year criminal background check within 90 days of start, including FBI and BCI fingerprinting
Monthly monitoring through EPLS/GSA/SAM and HHS/OIG databases
Education accreditation verification required
Fingerprinting must be completed using specific Ohio codes and within 1 year of start
Pending background check attestation required if fingerprint results are pending

Health & Immunization Requirements

Physical exam within 1 year of start confirming good health and free of contagious disease
Adult Tdap vaccination (after age 11/12) required
Negative TB test or chest X-ray within 1 year of start; TB questionnaire required annually
MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer required
Hepatitis B: Titer or 3-vaccine series (declination accepted)
Varicella: 2 vaccines or positive titer required unless medically exempt
Negative 10-panel drug screen within 3 months of start (no pending tests allowed)
Seasonal flu vaccine or declination during flu season (Oct 1 - Mar 31)
COVID vaccination not required but strongly recommended

Additional Requirements

2 professional references required (at least 1 from a manager or formal leadership)
Employment application and skills checklist required
Annual acknowledgment of dress code, hand hygiene, ethics, and compliance policies
Fraud, Waste, and Abuse training certificate required prior to start and updated annually
Unit-specific competency exams may be required
