Job Description

• Work schedule: Monday to Friday, 8:00 AM – 4:30 PM and/or 9:00 AM – 5:30 PM depending on department needs
• Required to float between outpatient and inpatient settings as needed, within scope of practice
• Weekend requirement: One Saturday per month
• Local candidates accepted
• No pending licenses accepted
• Time off restrictions: No holiday time off; maximum 5 total days off during the 13-week assignment. No additional time off approved after contract start. No time off approved the day before, the day of, or the day after a holiday.
• Attendance policy: Zero-fault attendance policy; missed shifts count as unexcused. Two missed shifts result in a warning; three or more may result in contract cancellation.

Requirements/Qualifications

• Valid Physical Therapy Assistant license in Oklahoma
• Minimum 1 year of experience as a Physical Therapy Assistant
• Experience as a Physical Therapy Assistant within the last year
• Minimum 1 year of experience in an outpatient setting
• Minimum 1 year of acute care experience
• Minimum 1 year of outpatient experience

Required Certifications & Licenses

• State Physical Therapist Assistant license (Primary source verification required prior to start)
• Basic Life Support (BLS) certification (AHA or American Red Cross accepted; RQI accepted)
• Advanced Cardiac Life Support (ACLS) required for certain units (Tele/PCU, ER, ICU/CCU, PACU, OR) – preferred but not required for observation units
• Other certifications may be required depending on unit assignment (e.g., Pediatric Advanced Life Support, Neonatal Resuscitation, Electronic Fetal Monitoring) – verify based on specific assignment

Required Documentation & Compliance

• Background check: 7-year criminal search including all counties worked in last 7 years, sex offender registry, nationwide social security trace; no exceptions for hits on background check
• Copy of diploma or transcript required (unofficial transcripts acceptable if graduation date is listed)
• Education verification required if transcript/diploma cannot be provided
• Physical exam within one year prior to start, stating "cleared to work" with physician signature
• Negative TB test or chest X-ray within one year (or chest X-ray within 5 years if prior positive TB)
• Required immunizations or titers: Measles, Mumps, Rubella, Varicella, Hepatitis B (series or positive titer or signed declination)
• Drug test: 10-panel drug screen within 30 days of start with negative results
• Flu vaccination or approved medical/religious exemption required during flu season (Oct 1 – May 1); documentation due by November 1
• Two professional references required prior to start
• Annual skills checklist and performance evaluations may be required if applicable
• Completion of facility-specific training modules prior to start (e.g., Oracle modules, WorkRamp modules)
• Badge photo required before start date (plain white background, clear front-facing photo, no filters)
• Various facility-specific agreements and checklists must be signed prior to start

Additional Information

• Must commit to completing lab requirements within 48-72 hours of signing contract
• Must adhere to strict attendance and time-off policies
• Floating between inpatient and outpatient settings is mandatory as needed
