Job Opportunity: Occupational Therapist - Home Health

Location: Medford, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: 8-hour day shifts (no 4x10 or 3x12 shifts available)
  • Patient Ratio: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
  • Start Date: ASAP
  • Weekend Requirement: No
  • On-Call: No
  • Floating: No
  • Block Schedule: Not accommodated
  • Locals Accepted: Yes (within 50-75 mile radius)
  • Pending License Accepted: Yes
  • Guaranteed Hours: Facility may call off up to one shift every two weeks
  • Transportation: Traveler must have own transportation; occasional travel radius up to 50+ miles
  • COVID-19 Vaccination: Primary series plus one booster required
  • Charting System: EPIC/Oasis
  • Attire: Any color scrubs
  • Parking: Free
  • Mileage Reimbursement: IRS standard

Required Qualifications

  • Valid Occupational Therapist license in Oregon
  • Minimum 1 year of Occupational Therapist experience within the last year
  • Experience in a Home Health setting
  • Experience with OASIS documentation
  • Basic Life Support (BLS) certification

Health and Compliance Requirements

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO
  • Fit mask testing required annually (N95 or equivalent)
  • Immunizations and/or documentation required:
    • COVID-19 vaccination (primary series + booster)
    • Flu vaccine during flu season or signed declination
    • Tetanus/Diphtheria/Tdap within 10 years or declination
    • Negative TB test or chest x-ray within 12 months, updated annually
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
    • Varicella vaccination or positive titer or declination
    • Hepatitis B vaccination series or signed declination
  • Drug screening: 9-panel drug screen within 12 months of start
  • Respiratory safety clearance annually for respirator use

Background and Documentation

  • Background check within 90 days covering all locations lived/worked in past 7 years
  • Education verification as applicable
  • Employment application covering at least last 5 years
  • Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting
  • State-specific searches and attestations as required

Additional Notes

  • Facility cannot accommodate alternative shift patterns such as 4x10 or 3x12
  • Traveler may be required to comply with float policies within a reasonable distance (no floating expected for this role)
  • Modules and training compliance required annually via the employer’s LMS system (WorkRamp)
  • Must comply with all facility-specific regulatory and specialty-specific training requirements
  • Traveler must be prepared to complete any required Epic training modules as assigned
