Physical Therapist - Home Health
Location: Petaluma, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Schedule: Monday to Friday, 8:30 AM – 5:00 PM with a 30-minute lunch break
- On-Call: Not required
- Territory: Marin County, covering a 50-mile radius from the office/hub
- Patient Load: 5-7 patients per day
- Types of Cases: Home Health - Geriatric
- Productivity Expectation: 30 productivity points per week
- EMR System: Experience required with HCHB
- OASIS Experience: Required
Required Qualifications
- Valid Physical Therapist license in California (must be in hand; pending licenses not accepted)
- Minimum 1 year of Physical Therapist experience within the last year
- Minimum 1 year of experience in a Home Health setting
- Experience with OASIS
- BLS Certification
- Prior travel experience: Not accepted for first-timers (first-time travelers not accepted)
Additional Information
- Mileage reimbursed at IRS standard rate between patient homes
- Local candidates accepted at travel rate
