Job Opportunity: Physical Therapist - Home Health
Location: Santa Rosa, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Specialty: Home Health Physical Therapy
- Minimum 1 year recent experience required in specialty
- Oasis and HCHB EMR experience required
- Average patients per day: 5-7 (to be confirmed during interview)
- Minimum 30 points weekly
- Guaranteed 40 hours per week
- Types of cases: Geriatric
- First-time travelers not accepted
- Must have license in hand and valid for California
- On-call required (details to be discussed during interview)
- Territory covers up to a 60-mile radius from the office hub
- Locals accepted at travel rate
- Traveler expected to be flexible with territory
- All charting must be completed by 9 AM the following morning
- Timely communication required
- Dress code: Scrubs (any color), no jeans
- Cell phone use prohibited; iPad provided
- Mileage reimbursed at IRS standard rate
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 1 year recent experience in Physical Therapy Home Health
- BLS (Basic Life Support) certification required (AHA, ARC, Military accepted)
Additional Requirements & Compliance
- Background checks within 90 days of start, including SSN trace, county searches, FACIS Level 1, nationwide sex offender, motor vehicle records, education and work history verification
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting requirements vary by state (Alaska, Connecticut, Washington)
- Education verification of highest degree related to profession
- State-specific criminal history and employability status checks as applicable
- Signed Orientation Safety Manual Acknowledgement (updated annually)
- Primary source verification of state license within 90 days of start and upon expiration
- Medication test/pharmacology competency exam as applicable
- Proof of current auto insurance required due to driving responsibilities
- Facility-specific acceptable use policy and flu/COVID vaccination or declination documentation required
- Physical exam within last 12 months (not required annually)
- Fit mask test (N95 accepted)
- Negative TB test or chest X-ray within 1 year (or TB questionnaire if applicable)
- Required immunizations or proof of immunity: Measles, Mumps, Rubella, Hepatitis B, Varicella
- 9-panel drug screen within last 12 months
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
- Two professional references within 2 years of start (travel, allied, per diem acute care)
- Full 7 years of work history documented with no gaps
- Annual skills checklist and work history verification within 90 days of start
Notes
- Prior travel experience is required; first-time travelers will not be considered
- Flexibility with travel territory and schedule is expected
- Strict adherence to documentation, communication, and compliance policies is mandatory
