Job Opportunity: Speech-Language Pathologist (Home Health)
Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 4 to 6 months (less experienced clinicians must commit to 6 months)
Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 5:00 PM)
Hours per Week: 40 hours guaranteed (minimum 23-24 patient visits per week required)
Job Description
- Provide home health therapy services primarily in counties including Alameda, San Joaquin/Stanilaus, Santa Clara, Sonoma, and Marin.
- Patient caseload focuses on home health, wound care, and geriatric patients.
- Emphasis on quality patient care over quantity of patients per day.
- No weekend work required.
- On-call duties are discretionary and not required.
- Mileage reimbursed for travel between patients (IRS standard), but not for travel to/from home.
- Local candidates within a 50-mile radius are accepted.
- Dress code: Any color scrubs and closed-toe shoes.
- Charting must be completed onsite using Kinnser/Wellsky systems; no remote charting allowed.
- Free residential parking available.
Required Qualifications
- Valid Speech Language Pathologist license in California.
- Minimum 1 year of Speech Language Pathologist experience within the last year.
- Experience in a Home Health setting.
- Basic Life Support (BLS) certification required.

Compliance and Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID).
- Emergency contact form prior to start.
- Background checks within 1 year, including SS trace, county searches (7 years), nationwide, and sex offender registry.
- Annual EPLS/GSA/SAM and HHS/OIG checks.
- Certifications: BLS required; ACLS and PALS preferred if applicable.
- State license primary source verification.
- Physical exam within 6 months signed by MD/DO confirming freedom from active communicable diseases.
- Immunizations/vaccinations or exemptions required for:
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Measles, Mumps, Rubella (proof of immunity or vaccination)
- Hepatitis B (titer, vaccination series, or declination)
- Varicella (titer, vaccination, history of chickenpox, or declination)
- Negative TB test or chest X-ray within 1 year (with special rules for past positives)
- Drug screening: 9-panel drug test within 1 year of start (instant acceptable).
- Seasonal flu vaccination or facility-specific declination form required during flu season (Oct 1 – Mar 31).
- COVID-19 vaccination or approved medical/religious exemption form required.
- Auto insurance card required for candidates using personal vehicles.
Additional Notes
- Territory and county assignments will be discussed during the interview; flexibility to cover surrounding counties may be required.
- All required return-to-office (RTO) documentation must be submitted at time of application; no additions accepted after offer.
- References: 2 professional references required for travel, allied, or per diem acute care clinicians; 1 reference for per diem non-acute care. Not required for non-clinical roles.
