Required Qualifications
- Valid Oregon RN license
- Associate of Science in Nursing or Associate Degree in Nursing
- BLS certification issued by the American Heart Association
- 1 year of oncology experience within the last year
- Orthopedics experience
- Nurse Navigator experience
Preferred Qualifications
- Recent ambulatory orthopedic experience
- Nurse Navigator or Care Manager experience
- Telephone triage/advice experience
Role Overview
- Coordinates care for orthopedic patients from pre-consult through surgery and recovery
- Serves as the single point of contact for pre-, intra-, and post-operative care coordination
- Identifies patient barriers and supports transitions of care
- Provides patient and family education and support
- Tracks referral and outcome data
- Collaborates with interdisciplinary teams and quality staff

- Employment application with at least 7 years of work history
- Two manager or supervisor references from the last 24 months
- Physical exam within 1 year of start
- Fit mask testing within 1 year prior to start
- Negative TB test or chest X-ray
- TB questionnaire
- Tdap vaccination or declination
- Measles, mumps, rubella, varicella, and hepatitis B immunity/vaccination or declination
- Negative 10-panel drug screen with methamphetamines listed out, completed within 3 months prior to start
- Flu vaccination or declination
- Signed facility documents and orientation materials before start
