Medical Surgical LVN / LPN Travel/Contract Position
Location: Chicago, Illinois
Contract Length: 13 weeks
Shift: Day (3 shifts of 12 hours each, 11:00 AM – 11:30 PM)
Hours per Week: 36 hours
Job Description
- Work in an acute care or ambulatory care setting within a correctional facility environment.
- Mandatory floating to other units within the facility as needed.
- Weekend requirement: Every other weekend (2 weekends per month).
- Guaranteed 40 hours per week.
- Charting system used: Cerner.
- Scrub color: Navy Blue.
- Free parking available on a first-come, first-served basis.
Required Qualifications
- Valid LPN / LVN license in Illinois.
- Minimum 1 year of LPN / LVN experience within the last year.
- Medical-Surgical (Med Surg) experience.
- BLS certification issued by the American Heart Association.
Certifications & Competencies
- Medication Test/Pharmacology Competency Exam (required within 12 months prior to start if starting on/after 4/14/24).
- Color Blind Testing (required within 12 months prior to start if starting on/after 1/16/24).
- Competency Exam (required within 12 months prior to start if starting on/after 4/14/24).
Health & Safety Requirements
- Physical exam within 12 months prior to start, signed by PA, NP, DO, or MD, and updated annually.
- Fit mask test (N95 or equivalent) within 1 year prior to start, updated annually. Medical or religious exemptions require approval for onsite PAPR use.
- Negative TB test (QGold or TSPOT) within 12 months prior to start, updated annually. PPD tests are no longer accepted as of 12/15/2025. If positive TB history, chest x-ray within 5 years and annual TB questionnaire required.
- Tdap vaccine within 10 years or signed declination within 12 months prior to start.
- MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer; low titer requires 1 booster.
- Hepatitis B series, titer, or signed declination within 1 year prior to start.
- Varicella: 2 vaccines or positive titer; low titer requires 1 booster.
- Negative 9-panel drug screen within 90 days prior to start, updated annually. Testing must be initiated within 48 hours of contract signing. Marijuana testing is not required.
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (10/01 – 5/4). Declinations are not accepted by the facility but must be collected for state requirements.
Background & Screening Requirements
- Background check within 12 months prior to start covering all locations lived/worked/schooled in past 7 years, updated annually. Includes SSN trace, nationwide and state-specific searches, sex offender registry, and more. Illinois fingerprinting required if indicated.
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start.
- Illinois Healthcare Worker Registry search required for aides and nursing assistants; must be listed as "Work Eligibility: Eligible." Not required for LVNs/LPNs.
- Drug testing and other health screenings as noted above.
Additional Information
- No call requirements.
- Request Time Off (RTO) policy: Maximum 5 days off with no more than 2 consecutive days; no RTO on holidays, weekends, or the day before/after holidays.
- Must complete facility-specific forms and rules of behavior prior to start.
- References and employment application required per state and agency standards.
- Must comply with mandatory floating policy within the facility.
