Contract Length: 13 weeks
Job Details
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 4:30 PM)
- Typical Patient Load: 1:1 ratio, may visit up to 6 patients per day
- Productivity Standard: 30 productivity points per day
- Weekend Requirement: No weekends required
- On-Call Requirement: No on-call duties
- Floating: No floating between units required
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Pending License: Accepted
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Traveler must have their own transportation
- Travel Radius: Up to approximately 60 miles per day (60-90 minutes total drive time)
- COVID-19 Vaccination: Primary series plus one booster required

Required Qualifications
- Valid Physical Therapy Assistant license in Oregon
- Minimum 1 year of Physical Therapy Assistant experience, including experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
Health and Safety Requirements
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation
- Immunizations/Vaccinations:
- COVID-19 primary series plus booster (required)
- Tetanus/Diphtheria/Tdap within 10 years (declination acceptable)
- Flu vaccine during flu season (declination acceptable)
- Measles, Mumps, Rubella (2 MMR vaccines or positive titer; declination acceptable)
- Varicella (2 vaccines or positive titer; declination acceptable)
- Hepatitis B (vaccination series or signed declination)
- TB Test: Negative TB test or chest X-ray within 12 months, updated annually
- Drug Screen: 9-panel drug screen within 12 months of start (instant acceptable)
- Respirator Clearance: Required annually for travel candidates; completed via virtual questionnaire
- Fit Mask Test: Required annually for travel candidates; may be completed onsite
Background and Compliance
- Background Check: Within 90 days of start, covering all locations lived/worked in past 7 years; must include SSN trace, death master index, sex offender registry, motor vehicle report, and state-specific abuse/neglect searches as applicable
- Education Verification: Required for all candidates
- State-Specific Searches: Required for Oregon and Washington border facilities as applicable
- Work History Verification: Required if reference requirements are not met; minimum 5 years verified work history
- References: Must be from supervisors; charge RNs acceptable if directly supervised

- Scheduling: Facility cannot accommodate 4x10 or 3x12 shifts
- Block Scheduling: Not available
- Floating Policy: Not required for this position
- Local Candidates: Accepted and considered at the same rate as travelers
