Registered Nurse - Home Health (Travel/Contract)
Location: Thomasville, North Carolina
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40 hours
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with an average patient load of 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, and Follow-up Visits.
- Must be willing to travel up to 75 miles from the home clinic; travel between patient homes is compensated (commute to/from first and last patient is not billable).
- Use of electronic medical records systems: HomeCare HomeBase (HCHB), OASIS (required proficiency), and Outlook.
- Dress code: Scrubs.
Required Qualifications
- Minimum 1 full year of recent skilled Adult Home Health nursing experience with OASIS completion or 1 full year Hospice experience depending on specialty.
- CPR and current BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Must have a current, valid nursing license for North Carolina (license in hand, no pending licenses accepted).
- Reliable transportation, current driver’s license, and agency-required liability insurance required.
- Proficiency with HomeCare HomeBase within the last 6 months; if last used over 6 months ago, retraining is required.
- First-time travelers not accepted; recent specialty experience is mandatory.
- Local candidates accepted at the same rate.
- Weekend and on-call requirements vary; discuss during interview.
Productivity Expectations
- Weekly productivity units: 25-30 points (15 points if part-time).
- Patient visits typically equate to 6-8 points per day.
Travel and Schedule Details
- Travel radius: up to 75 miles from home clinic.
- Typical schedule: Monday through Friday, 8 AM to 5 PM.
- Some positions may follow a 4x10 schedule (Friday-Monday), especially in western territories.
- Part-time schedule may be requested initially with potential to increase hours.
Compliance and Documentation Requirements
- Must complete a variety of background checks including:
- 7-year standard background check (including SS trace, county, nationwide, sex offender, state-specific searches).
- EPLS/GSA/SAM and HHS/OIG checks within 14 days of start, updated monthly.
- Fingerprinting as required by state (NC SBI Fingerprint Results for North Carolina).
- Education verification required for nursing degree.
- Two professional references required.
- Work history verification for at least two most recent employers or last 7 years.
- Must complete drug screening (10-panel).
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof required for:
- Measles (Rubeola), Mumps, Rubella (MMR)
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
- Seasonal flu vaccination or documented declination
- COVID-19 vaccination series or declination
- Must complete agency-required compliance training and review policies prior to start, including:
- HIPAA and Privacy Policy
- Code of Conduct
- Confidentiality agreements
- Home Health Core Policy and Scenarios
- COVID-19 Policy and Attestation
- Must have personal or auto insurance policy under candidate’s name or list candidate as a driver, current and not expired.
