Facility-specific modules must be completed within the first two weeks of the assignment
Candidate must complete the Ishihara color vision exam before start
Fit testing is required within 12 months of start and annually thereafter
Physical exam is required within 1 year of start and annually thereafter
TB screening is required within 1 year of start and annually thereafter
Drug testing is required within 30 days of start and annually thereafter
Tdap is required within 10 years of start or a signed declination within 1 year of start
COVID vaccination proof or signed declination is required for clearance
Two references from the last two years are required, with one from employment within the last year
