Role Focus

• Position is for an OT or PT specializing in hand therapy
• Certified Hand Therapist is preferred

Required Qualifications

• Valid Physical Therapist license in Oregon
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Experience in an Outpatient setting
• Occupational Therapist experience

Compliance / Pre-Start Requirements

• Active, unrestricted Oregon state professional license, if applicable
• BLS certification
• ACLS certification
• PALS certification
• Background check
• Education verification
• Orientation safety manual acknowledgement
• Competency exam
• Medication test/pharmacology competency exam
• Physical exam within 1 year prior to start
• Fit test on site
• Tdap vaccination within 10 years prior to start or signed declination
• TB screening within 1 year prior to start
• Measles, mumps, rubella, and varicella immunity documentation
• Hepatitis B immunity documentation, vaccine series, or signed declination
• Drug screening within 30 days prior to start
• At least 2 professional employment references
• Skills checklist
• Confidentiality and non-disclosure agreement
• Respiratory questionnaire
• Vaccine declination/waiver forms if declining required vaccines
• Photo for badge
• Traveler network account information form
• TDAP declination form if declining Tdap
• COVID exemption form if declining COVID vaccination requirements
• Compliance readiness through start date
