Licensed Practical Nurse (LPN) - Long Term Care

Location: Oneonta, New York
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 3:00 PM – 11:30 PM
Setting: Skilled Nursing Facility

Required Qualifications

Valid LPN / LVN license in New York
Minimum of 1 year of LPN / LVN experience, including experience within the last year
Basic Life Support (BLS) certification (required before onboarding)
Experience in Long Term Care settings
Completion of required health screenings and immunizations before onboarding:
- MMR
- Hepatitis B
- Influenza vaccine
- Tuberculosis (TB) test and annual TB questionnaire
- Varicella
- Fit test
- Physical/Health statement
Valid Driver’s License or State ID
Professional background checks and verifications including:
- Criminal background check
- Employment verification
- Education verification
- National and state sex offender registry checks
Completion of facility-specific requirements before onboarding:
- Two professional references
- Resume and skills checklist
- Core competencies exam
- Unit exam
- Skills checklist
- Drug screening
- Security credentials acknowledgment and confidentiality agreement
