Licensed Practical Nurse (LPN) - Long Term Care

Facility & Department Overview

• Facility type: Skilled Nursing Facility (SNF) and Long Term Care (LTC)
• Census: Typically 80-95 residents
• Room setup: 20 rooms are not private
• Electronic Medical Record (EMR) system: PCC
• Patient ratios:

  • LPN: 1:25 in both LTC and SNF
  • RN: 1:25 LTC, 1:13 SNF
  • CNA: 1:10-15 (aim to keep around 10)

Job Duties

• Administer IV antibiotics and fluids (no titratable drips)
• Perform patient assessments, medication administration, dressing changes, and wound vac care
• Support respiratory treatments when respiratory therapist is not available (RT on-site M-F during the day)
• CNAs handle vitals and activities of daily living (ADLs), but not blood sugars
• Admission and discharge processes are completed by RNs

Required Qualifications

• Licensed Practical Nurse (LPN) license valid in Illinois
• BLS (Basic Life Support) certification
• Valid driver’s license required for submission
• Minimum 1 year of nursing experience, preferably with geriatrics
• First-time travel nurses are welcome (prior travel experience not required)

Work Environment & Team

• Interdisciplinary team includes PT/OT/SLP, respiratory therapist, social worker, MDS nurses, wound care nurse, and dietician
• Respiratory therapy equipment includes AVAP/CPAP/BIPAP, mostly used at night
• Pharmacy available on-site 24/7 with Omnicell and medication carts
• Physician coverage: Hospitalist after hours, Nurse Practitioner during weekdays, plus on-call providers 24/7

Scheduling & Requirements

• Shift hours: 5:00 AM to 5:00 PM
• Weekend and holiday work: Every other weekend and holiday required
• No call or standby requirements
• Floating between units possible but generally assigned to the same unit during a stretch
• Orientation: One 12-hour shift on the floor; additional support available if needed
• Scrub color: Any professional color accepted

Safety & Infection Control

• Use of both PAPR and N95 masks required
• COVID patients are intermixed with other residents

This position offers a structured LTC environment with a supportive interdisciplinary team and a focus on geriatric care. It is suitable for LPNs with at least one year of experience, including those new to travel nursing.
