Job Opportunity: Medical Physicist

Location: Boise, Idaho
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Job Details

  • Shift: 7:00 AM – 7:00 PM
  • Experience with EPIC, ARIA RadOnc, SHARE DRIVE preferred
  • Certification by American Board of Radiology (ABR) required

Required Credentials and Onboarding Requirements

Certifications & Licenses

  • BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel.
  • License/Certificate Verification: Must be dated within 30 days of start. Online verification required.

Personal Health History

  • Fit Test
  • 10 Panel Drug Screen: Completed within 30 days of start, must include specific substances (no THC).
  • Color Vision Test
  • COVID-19 Vaccine: Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL).
  • MMR: Proof of 2 vaccinations or IgG titer lab report.
  • Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
  • Influenza Vaccine: Proof of vaccine, declination, or exemption for current flu season (declination not accepted in CA or IL).
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing with chest x-ray if positive (within last year or 5 years depending on region).
  • Varicella: Proof of 1 vaccination or IgG titer lab report.
  • Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
  • Tdap: Current, non-expired vaccine required; declinations accepted if no pediatric interaction.

Professional Requirements

  • Trinity Code of Conduct Form: Signed form required.
  • Confidentiality & Security Agreement: Signed form required.
  • Integrity and Audit Services Form: Signed shortly before start.
  • Motor Vehicle Report (MVR): Dated within 30 days of start, showing valid license.
  • Conflict of Interest Disclosure Form: Signed form required.
  • Criminal Background Check: Federal, nationwide, and county checks for last 7 years, including SSN verification, dated within 30 days of start.
  • Education Verification: Highest academic degree or diploma/GED verified.
  • EPLS (Excluded Parties List System) Check: Dated within 30 days of start.
  • GSA/OIG Exclusion Check: Dated within 30 days of start.
  • Joint Commission CORE Exam: Completed within last year.
  • NSO (National Sex Offender) Check: Dated within 30 days of start, including aliases and state-specific checks if applicable.
  • System Access Form: Must be completed and uploaded one week before start.
  • Skills Checklist: Provided by agency for clinical contractors.
