Job Opportunity: Occupational Therapist

Location: Aiea, Hawaii
Employment Type: Travel Contract
Contract Length: 12 weeks
Shift: Rotating (5 shifts per week, 8 hours each, 40 hours guaranteed)

Job Description

Provide occupational therapy services in a rotating shift schedule. This is a travel assignment requiring flexibility and adaptability to different shifts.

Required Qualifications

• Minimum 2 years of experience as an Occupational Therapist (OT)
• Active Hawaii OT License (or eligible under state executive orders for temporary licensure or licensure by endorsement)
• National Board for Certification in Occupational Therapy (NBCOT) certification
• American Heart Association Basic Life Support (AHA BLS) certification

Health and Compliance Requirements

• Proof of immunizations/vaccinations with no declinations allowed:

  • MMR (Measles, Mumps, Rubella)
  • Hepatitis B (must begin series if titer is low)
  • Influenza vaccine (required October 1 – March 31)
  • Varicella
  • Tdap
• Tuberculosis (TB) testing within 12 months prior to start date (Two Step Skin Test, TSpot, or Quantiferon)
• Annual TB questionnaire within past 12 months
• N-95 Fit Testing within the past 12 months with approved mask models
• Drug screen (10 panel) within 60 days prior to start
• 7-year criminal background check (county, state, national, federal) and SSN trace within 60 days prior to start
• Valid Driver’s License or State ID

Additional Requirements

• Completion of facility-specific core competencies, skills checklist, and unit exam
• Submission of professional references from recent assignments
• Verification of education and employment history
• Completion of confidentiality, dress code, orientation acknowledgement, latex questionnaire, traveler attestation, and other facility-required forms
