Physical Therapist - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (approximately 16 weeks)
Shift: 8:00 AM – 4:30 PM


Job Summary

Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes. Patient age range is from birth to over 100 years. Collaborate with patients, families, physicians, and other health professionals to assess needs and coordinate care. Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice. Weekend rotations occur once every 6 weeks. Ability to travel to assigned cases is required.


Required Qualifications

  • Valid Physical Therapist license in Massachusetts
  • Minimum of 1 year of experience as a Physical Therapist, including experience within the last year
  • Experience in a Home Health setting
  • Minimum of 1 year of Certified Home Care experience
  • Minimum of 1 year of Case Management experience

Onboarding and Compliance Requirements

  • Submission of a recent (within 6 months) professional color photo meeting specific guidelines (plain background, no glasses, symmetrical headshot).
  • Motor Vehicle Record (MVR) check within 45 days of start date.
  • Primary source verification of professional license within 30 days of start date and updated upon expiration.
  • N95 Respirator Fit Test within 365 days of start date, updated annually (no exemptions allowed).
  • Nurse Aide Registry check within 30 days of start date (required by MA Department of Public Health for all employees).
  • Respirator Medical Clearance attestation if fit test is required.
  • Documentation of immunity or vaccination for MMR (measles, mumps, rubella).
  • Tuberculosis (TB) screening: documentation of TB skin test or blood test within 3 months of start date, updated if there is a break in service longer than 6 months.
  • Physician-signed health statement within 365 days of start date, updated if break in service exceeds 6 months.
  • Criminal background check within 45 days prior to start date, including county and international residence/employment history for the past 7 years.
  • Massachusetts CORI criminal history check within 30 days prior to start date.
  • National sex offender registry check within 30 days prior to start date, updated annually.
  • Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days prior to start date, updated annually.
  • Verification of highest level of education through background check.
  • Social Security Number trace as part of criminal background check.
  • Submission of a detailed resume covering the past 7 years of work experience including city and state.
  • Two professional references within the last 4 years, including at least one supervisor or higher in the relevant skillset.
  • Completion of a skills checklist within 1 year of submission.
  • Confirmation of assignment form required for all initial assignments and extensions.

Additional Information

  • Positive TB test requires documentation of a negative chest x-ray after the positive test.
  • Hepatitis B vaccination or immunity is strongly preferred but optional.
  • Tdap vaccination within the last 10 years is preferred but optional.
  • Name change documentation required if legal name differs from credential or profile documentation.

This position requires a licensed Physical Therapist with home care experience, strong compliance with health and safety regulations, and the ability to travel to patient homes within the assigned region.
