Experience Requirements

Credentials & Certifications Required

- Influenza Vaccine: Required during flu season (Sept 15 - April 30), no exemptions allowed
- TB Test: Documentation of TB skin test or negative blood test within 3 months of start date, updated if >6 month break
- Health Statement: Physician statement within 365 days of start date, updated if >6 month break
- Hepatitis B: Strongly preferred (vaccination record or proof of immunity)
- Tdap: Preferred within last 10 years
- Background Checks:
- Criminal Background Check within 45 days prior to start
- Massachusetts CORI background check within 30 days prior to start
- SSN Trace covering last 7 years
- National Sex Offender Check within 30 days prior to start, updated annually
- Exclusion Checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start, updated annually
- Additional Documentation:
- Resume including past 7 years of work experience with city & state
- Two recent references (minimum one supervisor or above in skillset)
- Skills Checklist
- Candidate Submission Coversheet
- Confirmation of Assignment/POI Form upon acceptance and for extensions
- Respirator Medical Clearance Attestation (if fit test required)
Notes
- Photo submission required for badge: recent color photo with plain background, no filters or eyeglasses
- No religious or medical exemptions allowed for flu vaccine or fit testing
- Positive TB test requires negative chest x-ray documentation
- All credentials and health documentation must be current and compliant with hospital system policies
