Job Summary

• Provide physical therapy services to patients in their homes with physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
• Patient age range: birth to over 100 years.
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services.
• Maintain thorough and timely electronic documentation and interdisciplinary communication per agency policies and standards.
• Weekend rotations occur once every 6 weeks.
• Ability to travel to assigned cases is required.

Requirements

• Must hold a valid Physical Therapist professional license in Massachusetts.
• Minimum 1 year of home care experience required.

Contract Length

• 13 weeks

Onboarding & Credentialing Requirements

The following are required prior to or at onboarding (not necessarily at submission):

Licenses & Certifications

• Massachusetts Physical Therapist License (Primary source verification required; conditional licenses not accepted)
• Professional license and/or certification verification within 30 days of start and updated upon expiration

Background & Screening

• Criminal background check within 45 days prior to start, including county and international searches if applicable
• Massachusetts CORI (Criminal Offender Record Information) check within 30 days prior to start
• Social Security Number trace for past 7 years addresses
• National Sex Offender Registry check within 30 days prior to start and updated annually
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days prior to start and annually

Health & Immunizations

• N95 Fit Test within 365 days of start and updated annually (no religious exemptions)
• TB test documentation within 3 months of start (skin test or blood test) and updated if >6 month break
• Health statement signed by MD/DO/NP/PA within 365 days of start and updated if >6 month break
• Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions allowed
• MMR vaccination or proof of immunity
• Optional but strongly preferred: Hepatitis B vaccination or immunity proof
• Optional but preferred: Tdap vaccine within 10 years

Documentation & Other Requirements

• Resume including past 7 years of work experience with city and state
• Two recent references (minimum one supervisor or above) in relevant skillset
• Skills checklist specific to the role
• Candidate submission coversheet
• MGB Badge Photo (color photo, no selfies, plain background, no eyeglasses)
• Motor Vehicle Record (MVR) check within 45 days of start (required for all home care positions)
• Nurse Aide Registry check within 30 days of start (required by MA DPH for all employees)
• Respirator Medical Clearance Attestation (part of fit test record)
• Confirmation of Assignment/POI form for initial assignments and extensions
• Education verification of highest level achieved

Additional Notes

• No onsite fit testing; must be completed prior to start.
• Weekend rotations are infrequent (once every 6 weeks).
• Travel to patient homes is required.

This position is ideal for a licensed Physical Therapist with home care experience who is comfortable working independently in patient residences and meeting comprehensive credentialing and health requirements.
