Required Qualifications
- Valid RN license in Virginia
- Hospice (2 years of experience)
- Hospice experience within the last year
- One completed travel assignment
- BLS
- Associate of Science in Nursing / Associate Degree in Nursing
- Geriatrics experience
- Government-issued photo ID
- E-Verify or attestation
- 7-year background check within 30 days of start
- Virginia State Police background check
- EPLS/GSA/SAM within 30 days of start
- HHS/OIG within 30 days of start
- AHCA fingerprinting clearance on file prior to start
- NURSYS verification
- State license verification
- Contractor info form
- Contractor consent form signed prior to start
- Immunization statement completed prior to start
- Confidentiality and security agreement completed prior to start
- TB symptom survey completed prior to start
- Sexual abuse policy completed prior to start
- Code of conduct completed prior to start
- Photo for badge
- Conflict of interest form
- VA sworn statement
- Physical exam within 6 months of start
- Fit mask test within 1 year of start
- Negative TB test or chest x-ray
- Measles, mumps, rubella, hepatitis B, and varicella titers
- Negative 10-panel drug screen within 30 days of start
- Flu vaccine or declination
- COVID vaccine or declination
- Skills checklist 1 annually
Preferred
- ACLS, as applicable
- PALS, as applicable
- NRP, as applicable
- National tech certification verification, as applicable
- Medication test/pharmacology competency exam, as applicable
- Competency exam, as applicable
- Alzheimer’s training for PACE facilities, if applicable
- Tdap within 10 years of start for PACE facilities
