Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
ACLS - Advanced Cardiac Life Support issued by American Heart Association or American Red Cross
BLS - Basic Life Support issued by American Heart Association or American Red Cross
Experience with OASIS

On-call not required
Personal or auto insurance is required
Government-issued photo ID is required
Emergency contact form is required before start
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year of start
Measles, mumps, rubella, hepatitis B, varicella, and flu-season vaccination/documentation requirements apply
9-panel drug screen within 1 year of start
2 professional references within 3 years of start are required
Skills checklist required annually
