Pharmacist Inpatient - Travel/Contract

Location: San Francisco, California
Contract Length: 13 weeks

Job Description

• Provide patient-centered pharmaceutical care including clinical services, medication information, education, preparation, and distribution.
• Ensure safe, effective, and cost-efficient medication therapy.
• Use clinical judgment to implement patient care plans based on diagnosis.
• Communicate effectively with patients, families, and healthcare providers.
• Adhere to all local, state, and federal regulations, policies, and procedures to maintain privacy and safety.
• May perform specific tasks and orient new staff to the department.

Schedule

• Variable shifts:

  • Day: 4 shifts of 10 hours (7:00 AM - 5:30 PM)
  • Night: 5 shifts of 8 hours (9:00 PM - 5:30 AM)
• Weekend work every other weekend.
• No on-call required.

Required Qualifications

• Valid Hospital Pharmacist license in California.
• Minimum of 1 year of hospital pharmacist experience, with experience within the last year.
• Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree.

Background and Compliance Requirements

• 7-year background check including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
• EPLS/GSA/SAM and HHS/OIG checks required and updated annually.
• Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to California law).
• Employment application must include full 7 years of work history with no gaps.
• Work history verification required for up to 7 years; gaps over 30 days must be explained.
• Professional references: 2 required for clinical candidates within 3 years of hire.

Certifications

• ACLS (Advanced Cardiac Life Support) - AHA or ARC only.
• BLS (Basic Life Support) - AHA or ARC only.
• PALS (Pediatric Advanced Life Support) - AHA or ARC only.
• NRP (Neonatal Resuscitation Procedure) - AHA/AAP only, must be Instructor or Advanced Provider Course.
• State professional license verification required within 30 days of start.
• Medication/Pharmacology competency exam required as applicable.

Health and Immunization Requirements

• Physical exam within 1 year of start, including clearance for work.
• Fit mask test not required for pharmacists.
• Immunizations/vaccinations required:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest x-ray within 90 days of start, renewed annually.
  • Measles, Mumps, Rubella: Proof of immunity or vaccination series with boosters as needed.
  • Hepatitis B: Positive titer, series, or signed declination.
  • Varicella: Proof of immunity or vaccination series with boosters as needed.
  • Seasonal flu vaccination required during flu season (Nov 1 - Apr 30) or signed declination.
• Drug testing: Negative 9-panel drug screen within 6 months of start.

Facility Specific Requirements

• Candidate must review and sign facility policies including Standards of Business Conduct, Code of Conduct, Vendor Training, Overpayment Refund Policy, and General Regulatory Orientation prior to start.
• TB Screening Questionnaire signed by candidate and healthcare provider required within 90 days of start and renewed annually.
• Religious exemption forms required for facial hair (PAPR) or vaccine exemptions, subject to approval.

• Prior travel experience is not explicitly required.
• No on-call duties.
• New graduates are eligible.
• Must meet all background, health, and certification requirements prior to start.
