Location: Lacey, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Specialty: Physical Therapy (Home Health)
- Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
- Hours: 40 hours per week, guaranteed
- Average Patients per Day: 5-7 (to confirm during interview)
- Patient Population: Geriatric cases
- Territory: Mossyrock & Morton area (flexibility with territory expected)
- Dress Code: Scrubs of any color; jeans not allowed
- Device Provided: iPad (cell phone use not allowed)
- Mileage Reimbursement: Yes, at IRS standard rate
- On-call: Yes, every 6th weekend
- Locals: Not accepted
- Timely documentation and communication required
Required Qualifications
- Valid Physical Therapist license in Washington
- Minimum 1 year of Physical Therapist experience within the last year
- BLS (Basic Life Support) certification required (AHA, ARC, Military accepted)
- Experience with Homecare Homebase (HCHB) EMR
- Experience with OASIS documentation required
Compliance & Background Requirements
- Background checks within 90 days of start, including:
- SSN Trace, County searches (last 7 years), FACIS Level 1, Nationwide, Sex Offender, Motor Vehicle Records, Education Verification, Work History Verification (7 years)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
- Education verification of highest degree related to profession
- Signed Orientation Safety Manual Acknowledgement (updated annually)
- Agency Background Attestation required prior to start
- Proof of current auto insurance required (due to driving responsibilities)
Health & Immunization Requirements
- Physical exam within the last 12 months (not required annually)
- Fit test for N95 mask (no specific model required)
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
- Annual TB Questionnaire required (especially for positive TB tests)
- Immunizations or positive titers for:
- Measles (Rubeola), Mumps, Rubella (2 vaccines or booster/declination; candidates born before 1957 exempt)
- Hepatitis B (series, titer, booster, or declination)
- Varicella (series, titer, booster, or declination)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
- 9-panel drug screen within 12 months (instant acceptable)
Additional Requirements
- 2 professional references within 2 years for travel/allied/per diem acute care roles
- Full 7 years of work history documented with no gaps in employment application
- Annual skills checklist and competency exams as applicable
- Must complete facility-specific Acceptable Use Policy prior to start
