Job Opportunity: Pharmacist Inpatient (Travel/Contract)
Location: Madison, Wisconsin
Contract Length: 13 weeks
Job Details
- Employment Type: Travel/Contract
- Shift: Day (11:00 AM - 7:30 PM) or Evening (2:30 PM - 11:00 PM)
- Hours: 8-hour shifts, 40 hours per week
- Schedule: Every 3rd weekend rotation
- Setting: Inpatient pharmacy
Required Qualifications
- Valid Clinical Pharmacist license in Wisconsin
- Minimum 1 year of clinical pharmacist experience within the last year
- Doctor of Pharmacy (PharmD) degree
- Basic Life Support (BLS) certification
- Experience with pharmacokinetic dosing, renal dosing, and drug shortage management
- Experience with Epic electronic health record system
Important Requirements & Conditions
- Travelers must shave to ensure proper mask seal for fit testing; use of personal CAPRs or PAPRs is not allowed, and these are not provided by the facility
- Fit testing will be completed onsite during the first week of orientation; failure to comply may result in assignment cancellation
- Scrubs are provided
Compliance & Documentation
- Government-issued photo ID required
- Criminal background check within 12 months prior to start
- Negative drug screen within 12 months prior to start
- TB test (PPD, Qgold, or TSPOT) within 12 months; chest x-ray within 5 years if positive PPD
- Required immunizations or proof of immunity: Rubeola (Measles), Mumps, Rubella, Varicella, Hepatitis B (or declination), Flu seasonal vaccination or approved medical/religious exemption (must be renewed annually)
- COVID vaccination series preferred
- OSHA Respirator Clearance required prior to start for fit testing
- Completion of facility-specific orientation modules and post-start orientation checklists
- Physical exam within 1 year indicating good health
- Two professional references required (for new candidates)
Additional Notes
- Candidates will complete all required compliance modules annually
- Post-start orientation checklists must be completed and returned within the first week
- Facility-specific confidentiality, security agreements, and standards of ethical conduct must be reviewed and signed prior to start
