Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Home Health
Location: Lacey, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday (8:00 AM – 4:30 PM), 40 hours per week
Schedule: Monday-Friday with some weekend flexibility required
Average Patients per Day: 5-7 (to be confirmed during interview)
Territory: Up to 60-mile radius from the office; flexibility with territory expected
Dress Code: Scrubs (any color), no jeans
Technology Provided: iPad provided; cell phone use not allowed
Mileage: Reimbursed at IRS standard rate
Job Description
- Provide nursing care in a home health setting, primarily caring for geriatric patients.
- Timely documentation and communication are required.
- No on-call responsibilities.
- First-time travel nurses are not accepted for this position.
- Productivity requirement: 30 points weekly.
Required Qualifications
- Valid LPN/LVN license in Washington state.
- Minimum of 1 year of LPN/LVN experience within the last year.
- Experience in home health nursing.
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
- Experience with Homecare Homebase electronic medical record system.
Compliance and Background Requirements
- Background checks must be current within 90 days of start and updated annually, including:
- SSN Trace
- County searches for the last 7 years of residence/work/school
- FACIS Level 1
- Nationwide Sex Offender Registry
- Motor Vehicle Records/Driver’s Record
- Education verification
- Work history verification (7 years)
- State-specific background checks and fingerprinting as applicable for Washington.
- Employment application with full 7 years of work history, no gaps.
- Two professional references within 2 years of start.
Health and Safety Requirements
- Physical exam within the last 12 months (not required annually).
- Fit test for N95 mask (no specific model required).
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if previously positive. Annual TB questionnaire required.
- Immunizations or titers for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Seasonal flu vaccination or facility-specific declination form required for flu season 2025-26.
- COVID vaccination or facility-specific declination required.
- 9-panel drug screen within 12 months of start (instant acceptable).
Additional Requirements
- Proof of current auto insurance required due to driving responsibilities.
- Signed acknowledgment of Orientation Safety Manual and Acceptable Use Policy prior to start.
- Compliance with agency background attestation and facility-specific documentation.
Notes
- Locals are accepted and paid at travel rates.
- Prior travel experience is not required, but first-time travelers are not accepted.
- No cell phone use during work hours.
