Required Qualifications

  • Valid Occupational Therapist license in Utah
  • BLS certification
  • ACLS certification
  • PALS certification
  • National tech certification verification
  • Government-issued photo ID
  • E-Verify
  • Background check
  • References: 3 professional references, including at least 1 supervisor/manager or above
  • Annual skills checklist

Compliance / Health Requirements

  • Physical exam within 1 year of start
  • Tdap within 10 years of start
  • Negative TB test or chest X-ray
  • TB questionnaire if there is a past positive TB history
  • Measles, mumps, and rubella immunity documentation
  • Hepatitis B surface antibody titer showing immunity
  • Varicella immunity documentation
  • 11-panel drug screen including MDMA within 30 days of start
  • Flu vaccination or approved exemption
  • Respirator medical clearance if a respirator fit test is needed onsite
  • Orientation safety manual acknowledgement
  • Competency exam
  • Medication test/pharmacology competency exam

Background Screening

  • Background check
  • EPLS/GSA/SAM screening
  • HHS/OIG screening
