Job Opportunity: Hospice Registered Nurse (Travel/Contract)

Location: Lorain, Ohio
Schedule: Monday to Friday, 8:00 AM – 5:00 PM (Day Shift, 5x8 hours)
Contract Length: 4 weeks

Job Overview

• Specialty: Hospice Registered Nurse – Admissions
• Patient Load: Approximately 5-7 patients per day (geriatric cases)
• Guaranteed 40 hours per week
• On-call required (triage team handles calls; on-call only if a true visit is needed)
• Weekend coverage: On-call only
• Territory: Up to 60-mile radius from the office; flexibility with territory expected
• Dress Code: Scrubs of any color; no jeans allowed
• Technology Provided: iPad provided; cell phone use not allowed
• Mileage reimbursed at IRS standard rate
• First-time travel nurses accepted
• Locals accepted at travel rate

Required Qualifications

• Valid RN license in Ohio
• Minimum 1 year of hospice experience within the last year
• BLS certification issued by American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience with Homecare Homebase (HCHB) EMR system

Background and Compliance Requirements

• Background checks required within 90 days of start, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification
  • State-specific searches as applicable (e.g., Kentucky, Virginia, Indiana, New Hampshire)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting required for Ohio placements (BCI and FBI), must be current within 12 months; reprints required if candidate has lived/worked outside Ohio since last prints
• Education verification of highest degree related to profession required
• NURSYS verification for RN license within 90 days of start
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Medication/Pharmacology competency exam as applicable
• Agency background attestation required prior to start

Health and Immunization Requirements

• Physical exam within the last 12 months (physician statement)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
• Annual TB questionnaire required
• Immunizations or proof of immunity for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 12 months of start (instant acceptable)
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Documentation and History

• 7 years of full work history with no gaps, including city and state of employment
• Work history verification within 90 days of start (3 attempts required if unverifiable)
• Two professional references required within 2 years of start (travel, allied, per diem acute care)
• Skills checklist required annually

Additional Notes

• Proof of current auto insurance required if job duties include driving
• Candidates must complete facility-specific Acceptable Use Policy prior to start
• Candidates must complete facility-specific flu and COVID declination forms if applicable

This position is ideal for a Hospice RN with recent experience, flexibility for travel within a 60-mile radius, and the ability to manage geriatric hospice patients in a home health setting.
