Job Description

• Specialty: Medical Surgical / LPN
• IV Certification: Firm requirement
• Shift Options: 48-hour or 36-hour shifts (indicate preference)
• Start Date: ASAP
• Nurse-to-Patient Ratio: Approximately 1:7 (team nursing with support from PCC, RN, etc.)
• On-Call: As needed
• Weekend Requirement: As needed
• Block Scheduling: Not open to block schedules; weekend blocks may be considered
• Locals: Not accepted (strict 100-mile radius rule)
• Pending License: Not accepted; must comply with Nurse Licensure Compact (NLC) guidelines
• Return to Office (RTO) Restrictions: Must be included at submission; no changes after offer

Experience Requirements

• Minimum 2 years recent experience in LVN/LPN specialty
• First-time travelers not accepted unless they have 3+ years recent experience
• Prior travel experience required
• Level I Trauma experience required
• Large hospital experience required

Certifications & Licenses Required

• Current LVN/LPN license (TN or multistate) compliant with NLC
• IV Certification (mandatory)
• BLS (Basic Life Support) - AHA or ARC only
• ACLS (Advanced Cardiac Life Support) - required for Medical Surgical/Tele units
• Other certifications may be required depending on unit assignment (e.g., PALS, NRP, TNCC) but not specified for this role

Compliance & Documentation Requirements

• Background Check within 60 days of start, including statewide, county, federal, and education verification
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and annually thereafter
• Education verification (diploma/transcript not accepted in lieu)
• Sex Offender Search and Tennessee Adult Abuse Registry Search (TN only)
• Physical exam within 1 year of start, attesting ability to meet job demands
• Negative 10-panel drug test within 30 days of start (no pending results accepted)
• Immunizations:

  • Measles, Mumps, Rubella (proof of 2 vaccines or positive titer with boosters if needed)
  • Varicella (proof of 2 vaccines or positive titer with boosters if needed)
  • Hepatitis B (3-shot series, positive titer, or signed declination)
  • Flu vaccine or approved medical/religious exemption required during flu season (declinations not accepted)
• TB testing: True 2-step PPD, Q-Gold, or T-spot within 1 year, updated annually; chest X-ray if positive
• Fit Mask Test: 3M N-95 mask fit test within 1 year, mandatory

Additional Requirements

• Two recent professional references required
• Skills checklist and internal evaluations required for candidates employed with agency for 1+ year
• Facility-specific dress code: Navy blue scrubs for RNs; no prints or embroidered scrubs allowed
• Photo badge required prior to start (strict guidelines on photo quality and appearance)
