Registered Nurse - Home Health (Travel/Contract)

Location: South Portland, Maine
Contract Length: 13 weeks

Job Overview

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conduct home visits with periodic office meetings.
• Average patient load: 5-8 patients per day.
• Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
• Must be willing to travel up to 75 miles from home clinic; average weekly travel 250-350 miles.
• Use of HomeCare HomeBase (HCHB) EMR required; proficiency in Oasis is also required.

Schedule

• Monday to Friday, 8:00 AM to 5:00 PM
• No weekend rotation or on-call required for this assignment.
• Some positions may have a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8.

Experience & Requirements

• Minimum 1 full year of recent Adult Home Health experience OR 1 full year Hospice experience (whichever is higher).
• First-time travelers NOT accepted; must have recent specialty experience.
• CPR and BLS certification required.
• Must have a current nursing license in hand (no pending licenses accepted).
• Reliable transportation and current driver's license required.
• Local candidates accepted at the same rate.

Documentation & Compliance

• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required; retraining may be required if last use was over 6 months ago.
• Must complete various compliance and facility-specific documents prior to start, including but not limited to:

  • Background checks (7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG)
  • Fingerprinting (state-specific requirements apply)
  • Education verification
  • Physical exam within 12 months
  • Negative TB test or chest X-ray within 12 months
  • Immunizations and titers (Measles, Mumps, Rubella, Hepatitis B, Varicella)
  • Drug screening (10 panel)
  • COVID-19 vaccination or declination
  • Various state-specific and facility-specific attestations and policy acknowledgments
• Must have personal or auto insurance policy under candidate's name or be listed as a driver.

Additional Notes

• Productivity expectations: 30 points per week (points assigned based on visits and activities).
• If unable to meet financial needs due to low patient census, floating to a nearby clinic may be discussed.
• Dress code: Scrubs.
• Prior travel experience is NOT explicitly required for this position.
