Key Requirements and Compliance
Background Checks
- Must be completed within 90 days prior to start date.
- Includes SSN trace, county, nationwide, sex offender checks for all locations lived/worked/schooled in past 7 years.
- Background checks with pending charges or convictions require a facility review before start.
- Monthly monitoring through workforce monitoring systems (EPLS/GSA/SAM, HHS/OIG).
- Wisconsin Caregiver Screening required within 90 days of start.
Education Verification
- Strict education verification required based on specialty.
- Specific professions require both education verification and high school diploma/GED verification (e.g., Medical Assistant, Endoscopy Tech).
- Hard copies of diplomas or transcripts may be accepted if verification is difficult.
Certifications
- ACLS, BLS, PALS, NRP certifications required as applicable, issued by American Heart Association.
- National certification verification required within 30 days of start and upon expiration.
- State license verification and NURSYS verification (for RN, LPN/LVN, AP) required within 30 days of start and upon expiration.
- Orientation Safety Manual Acknowledgement and Pass/Fail Telemetry Testing required for applicable roles.
- Telemetry exams (Adult and Pediatric) must be completed prior to start and annually, with remediation process for failures.
Competency Exams
- Medication/Pharmacology competency exams required as applicable.
- Color Blind Testing required within 12 months for Laboratory or Dialysis roles.
- Other competency exams required based on profession/specialty.
Health Documents and Immunizations
- Physical exam (Aya standard) required within 1 year.
- Fit mask testing completed onsite.
- Tetanus/Diphtheria/Tdap vaccination proof required if working in certain pediatric areas.
- Negative TB test or chest x-ray required with specific acceptable test types.
- Annual TB questionnaire required.
- MMR (Measles, Mumps, Rubella) vaccination or positive titers required; medical or religious exemptions allowed with approval.
- Hepatitis B vaccination series and positive titer or declination form required for direct patient care roles.
- Varicella vaccination or positive titer required; medical or religious exemptions allowed.
- Drug testing required for clinical candidates; pharmacy department candidates require specific drug screen within 45 days of start.
- Seasonal flu vaccination or approved medical/religious exemption required annually.
- Meningococcal vaccine or declination required for Microbiology department staff.
Facility Specific Documents
- Passport-style photo for badge required prior to start.
- Communicable Disease Form to be completed within 90 days of start.
- Flu, MMR, Varicella medical and religious exemption forms as applicable.
- Telemetry test study guides and remediation courses available for required exams.
- Hepatitis B declination form if applicable.
