Position Details

• Specialty: Therapy/Rehabilitation (Pediatric Physical Therapy)
• Shift: Monday to Friday, 5 days a week, 8-hour shifts (9:30 AM - 6:00 PM)
• Shift Options: Any 8 hours between 7:30 AM and 6:00 PM
• Start Date: ASAP
• Contract Length: 13 weeks

• Local Radius: 50 miles (locals accepted)
• Guaranteed Hours: Facility may call off 1 shift per 2 weeks

• Additional Requirements: Pediatric and infant population experience is mandatory (hard stop)

Required Certifications & Licenses

Additional certifications may be required based on clinical track and facility needs, including but not limited to:

• ACLS (Advanced Cardiac Life Support)
• PALS (Pediatric Advanced Life Support)
• NRP (Neonatal Resuscitation Program)
• Other specialty certifications as applicable

Compliance & Documentation Requirements

• Background Check: Must be completed within 45 days of start, covering all locations lived/worked in the past 7 years. Includes SSN trace, death master index, sex offender registry, education verification, motor vehicle record (if applicable), and international background check if applicable.
• Drug Testing: 10-panel drug screen within 90 days of start (no instant tests).
• Health Documents:

  • Physical exam within 1 year of start
  • Immunizations: TDAP, MMR (Measles, Mumps, Rubella), Varicella, Hepatitis B (or declination), Flu vaccine (seasonal), COVID vaccine or declination
  • Negative TB test or chest X-ray within 12 months
  • Fit mask test (N95 or equivalent) required for travel starts after 2/5/2024
  • Respiratory safety clearance for respirator use (travel only)
• References: Minimum 2 supervisor references covering 1 year of employment within the last 3 years in a comparable specialty/setting
• Employment Application: Must include at least last 5 years of employment with month/year format
• Skills Checklist: Required for applicable units, updated annually

Training & Competency

• Completion of Providence Health and Services required WorkRamp modules prior to start, including:

  • Core modules for profession
  • Specialty-specific modules if applicable (e.g., Pediatric Therapy)
  • Annual regulatory requirements
• Competency exams as applicable (e.g., medication test/pharmacology)
• Epic training modules completion required for charting system access

Additional Notes

• All requested time off (RTO) must be submitted at time of application and approved based on facility needs.
• Facility may require floating to other Providence facilities within a reasonable distance.
• Pending state licenses are not accepted.
• Candidates must comply with all Providence Health and Services policies and procedures.

This position is ideal for a Pediatric Physical Therapist with at least 1 year of experience, comfortable working in an acute care hospital setting, and with strong pediatric and infant therapy skills.
