Job Opportunity: Physical Therapist (Home Health) - Travel/Contract

Job Description

• Provide Physical Therapy services in a Home Health setting, including home visits and periodic office meetings.
• Patient load: 5-10 patients per day, averaging 5-6 patients daily.
• Types of cases: Start of Care and Follow-Ups.
• Schedule: Monday to Friday, 8 AM to 5 PM.
• Visits include both patient homes and care facilities.
• Travel: Must be willing to travel up to 75 miles from the home clinic.
• Mileage reimbursed at IRS standard rate (patient home to patient home travel compensated; commute to/from first and last patient is not billable).
• On-call requirements vary; may include weekend coverage every other week for orthopedic patients.
• Some positions may follow a Baylor Shift schedule (4x10s, Friday-Monday), but most are standard 5x8 schedules.

Requirements

• Experience: Minimum 1 full year of recent Adult Home Health Physical Therapy experience.
• Prior Travel Experience: Not explicitly required.
• Certifications: CPR and BLS required.
• Licensure: Must have current state license in hand (pending licenses accepted only if license issuance is expected within a few days).
• Transportation: Reliable transportation and current driver’s license required.
• Insurance: Agency-required liability insurance required.
• Local Candidates: Accepted at the same rate.
• First-timers: Not accepted; must have recent Home Health experience.

Productivity & Tools

• Productivity expectations: 25-30 weekly units; workload approximately 30 points per 40 hours.
• EMR systems used: HomeCare HomeBase (HCHB), Oasis, Outlook.

Additional Notes

• Guaranteed hours are not provided; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.
• Weekend requirements and on-call duties vary by location and should be discussed during the interview.

• This is a true contract assignment with potential for hire at the end of the contract.
