Job Opportunity: Physical Therapist (PT) - Home Health

Location: Concord, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

Specialty: Home Health Physical Therapy
Territory: West Team, up to a 60-mile radius from the hub/office
Average Patients per Day: 5-7
Guaranteed Hours: 40 hours/week
Types of Cases: Geriatric
First-time travelers accepted
On-call Requirement: Yes, every 3rd to 4th weekend
Dress Code: Scrubs (any color), no jeans
Device Provided: iPad
Cell phone use: Not allowed
Mileage reimbursed at IRS standard rate
Flexibility with territory expected

Required Qualifications

Minimum 1 year recent Physical Therapy experience in Home Health
License: Physical Therapist license in hand (California)
Certifications:
- BLS (Basic Life Support) required
EMR Experience: HCHB experience required
OASIS experience preferred
Must be able to work on-call as required

Additional Requirements

Background checks within 1 year, including SS Trace, County searches, Nationwide, Sex Offender, EPLS/GSA/SAM, and HHS/OIG (annual updates required)
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year
Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella as applicable
Drug screening (9 panel) within 1 year
Flu vaccination or declination during flu season (Oct 1 - Mar 31)
Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
Annual skills checklist and competency exams as applicable

Notes

Locals are accepted at travel rates
Flexibility and willingness to travel within the territory are important
Scrubs required; no jeans allowed
Prior travel experience is NOT explicitly required for this position
