Job Opportunity: PT Home Health (Therapy/Rehabilitation)

Location: San Jose, California
Employment Type: Travel/Contract


Position Details

  • Specialty: Physical Therapist (PT) in Home Health
  • Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
  • Visits: Minimum 23-24 visits per week to qualify for a 40-hour guarantee
  • Work Schedule: No weekends required
  • Territory: May include surrounding counties; specifics discussed during interview
  • Locals Accepted: Yes, within a 50-mile radius
  • Dress Code: Any color scrubs, closed-toe shoes
  • Charting: Must be completed onsite using Kinnser/Wellsky; no charting from home
  • Parking: Free residential parking available

Experience & Requirements

  • Years of Experience:
    • Minimum 1 year in Home Health
    • Minimum 2 years overall as a clinician (higher value applies)
  • First-time Travelers: Accepted
  • Prior Travel Experience: Not required
  • Certifications Required:
    • BLS (Basic Life Support)
    • State License in hand at time of submission (no pending licenses accepted)
  • On-Call: Not required (discretionary)

Contract Length

  • 4-6 months per contract
  • Less experienced clinicians must commit to 6 months (non-negotiable)

Compliance & Documentation Requirements

  • All required documents must be completed and approved before starting patient care (compliance process may take 2-3 weeks)
  • Required documents include but are not limited to:
    • Government-issued photo ID (Driver’s License/State ID)
    • Background checks (including SS Trace, County searches, EPLS/GSA/SAM, HHS/OIG)
    • Certifications: BLS, ACLS, PALS as applicable
    • State License verification
    • Physical exam within 6 months (signed by MD/DO, free of active communicable diseases)
    • Immunizations and titers: Measles, Mumps, Rubella, Hepatitis B, Varicella, Tetanus/Diphtheria/Tdap
    • Negative TB test or chest X-ray within 1 year
    • Drug screening (9-panel) within 1 year
    • Flu vaccination or declination during flu season (Oct 1 - Mar 31)
    • COVID vaccination or approved medical/religious exemption
    • Emergency contact form
    • Auto insurance card (if applicable)
  • References:
    • 2 professional references for travel, allied, or per diem acute care
    • 1 professional reference for per diem non-acute care
  • Skills checklist required annually

Additional Notes

  • Mileage reimbursed based on IRS standard from first to last patient visit (not to/from home)
  • Return to Office (RTO) requests must be submitted at time of application; no additional RTO accepted after offer
  • Valued clinician qualities: patient care focused, punctual, self-starter, diligent with charting (tablet provided)
  • COVID vaccination required; declination allowed for religious reasons
  • Flu vaccination declination allowed with required form
