Preferred Certifications:

Physical exam within 1 year, updated annually
Fit mask test within 1 year (specific mask models required)
Tetanus/Diphtheria/Tdap vaccination or declination (within 2 business days of contract signing)
Negative TB test or chest X-ray (within 2 business days of contract signing)
TB questionnaire if positive TB test
Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (declination allowed)
- Varicella
Negative 9-panel drug test within 30 days of start
Seasonal flu vaccination/documentation or approved medical/religious exemption (flu season Oct-Mar)

Valid Driver’s License or State ID
7-year background check including aliases and multiple federal/state databases
EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
Education verification (ADN or BSN required)
State license primary source verification within 30 days of start
NURSYS verification for RN license within 30 days of start
Employment application with 7 years of work history and explanation of gaps over 30 days
Skills checklist updated annually for assigned and float units
Employment verification for 7 years (third party or in-house)
Two professional references required
Photo for badge (passport style, plain background)
Completion of facility orientation modules prior to start (approx. 2 hours)

Fingerprinting and addition to Illinois Department of Public Health (IDPH) Health Care Worker Registry required if position does not require an Illinois state license
Fingerprints must be completed in Illinois no later than the Monday before start date

Candidates must meet all certification and health requirements prior to start
Religious and medical exemption forms for COVID-19 and influenza vaccines require facility approval and must be submitted at least one week prior to documentation due date
First-time travel nurses are accepted for this position
