Job Opportunity: Medical Surgical Nurse (LVN / LPN)
Location: Chicago, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Shift: Night shift, 5 days per week, 8 hours per day (23:00 - 7:30)
- Guaranteed Hours: 40 hours per week
- Facility Type: Correctional health services
- Scrub Color: Navy Blue
- Charting System: Cerner
- Parking: Free, first come first serve
- Floating: Mandatory float within scope of practice as needed by the facility
- Weekend Requirement: Every other weekend (2 weekends per month)
- Request Time Off (RTO): Maximum 5 days, no more than 2 consecutive; no RTO on holidays, weekends, or the day before/after holidays
Required Qualifications
- Valid LPN / LVN license in Illinois
- Minimum 1 year of Long Term Care experience within the last year
- Experience in Acute Care setting
- Experience in Corrections setting
- Medical-Surgical nursing experience
- BLS certification issued by the American Heart Association
- Successful completion of coursework for medication administration (Pharmacology) as a Practical Nurse
Required Certifications & Competencies
- BLS certification (AHA only)
- Medication Test/Pharmacology Competency Exam (required for starts on/after 4/14/24; must be completed within 12 months of start)
- Color Blind Testing Online (required for starts on/after 1/16/24; must be completed within 12 months of start)
- Competency Exam (required for starts on/after 4/14/24; must be completed within 12 months of start)
Health & Safety Requirements
- Physical exam within 12 months of start, signed by PA, NP, DO, or MD
- Fit mask test within 1 year of start (specific N95 masks required); medical or religious exemptions require approval for onsite PAPR use
- Negative TB test (QGold or TSPOT) within 12 months of start, updated annually (PPD not accepted as of 12/15/2025)
- Tdap vaccine within 10 years or signed declination (declination must be within 12 months of start)
- MMR vaccines or positive titers for Rubeola, Mumps, and Rubella (boosters required for low titers)
- Hepatitis B series, titer, or signed declination (declination must be within 1 year of start)
- Varicella vaccines or positive titer (booster required for low titer)
- Negative 9-panel drug screen within 90 days of start, updated annually (marijuana not required)
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (10/1 to ~4/1)
Background & Documentation Requirements
- Government-issued photo ID (driver’s license or state ID)
- Background check within 12 months covering all locations lived/worked/schooled in past 7 years, updated annually
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- Illinois Healthcare Worker Registry verification for aides and nursing assistants (not required for LPNs)
- Various facility-specific forms and rules of behavior must be completed prior to start
- Non-employee/volunteer application with PREA clearance required and must be completed within 24 hours of contract signing
