Job Opportunity: Outpatient Physical Therapy Assistant (PTA)

Job Details

Required Qualifications

• Valid Physical Therapy Assistant license in Illinois
• Minimum 1 year of Physical Therapy Assistant experience within the last year
• BLS certification required

Prior Travel Experience

• First-timers not accepted, indicating prior travel experience is required

Required Certifications & Verifications

• BLS (Basic Life Support) - AHA or RQI acceptable
• Illinois PTA state license verification (primary source)
• Competency exam required for clinical/allied candidates
• Background checks including:

  • Criminal history, SSN trace, sex offender registry, FACIS Level 3
  • EPLS/GSA/SAM, HHS/OIG checks
  • Illinois Department of Corrections Inmate Search
  • Illinois Most Wanted and Violent Offender Registry searches
• Education verification (official or unofficial transcripts acceptable if pending)
• Physical exam within 1 year of start, confirming good health and ability to perform job functions
• Fit mask test required (3M 1860, 1870, or equivalent)
• Negative TB test or chest X-ray within 1 year
• Immunizations or proof of immunity for:

  • Measles (Rubeola), Mumps, Rubella (2 MMR vaccines or positive titer)
  • Hepatitis B vaccine series or positive titer
  • Varicella vaccine or positive titer
• 9-panel drug screen within 30 days of start
• Flu vaccination or approved medical/religious exemption required annually

Additional Requirements

• Photo for badge (color, mid-shoulders up, plain background) due before start
• Code of Conduct acknowledgment within 7 days of contract signing
• Parking permit form to be completed within 7 days of contract signing

Notes

• No pending licenses accepted; must have active Illinois PTA license
• Prior travel experience required (first-time travelers not accepted)
• Schedule is primarily Monday-Friday with no weekend or on-call requirements
• Facility uses Cerner for charting and requires gray scrubs or professional attire
