Job Opportunity: Medical Surgical Nurse (LVN / LPN)

Location: Chicago, Illinois
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: Day shift, 3 shifts per week, 12 hours each (11:00 AM – 11:30 PM)
  • Hours per Week: 36 hours
  • Guaranteed Hours: 40 hours per week
  • Facility Type: Correctional health services
  • Float Requirement: Mandatory floating within scope to meet facility needs
  • Weekend Requirement: Every other weekend (2 weekends per month)
  • Return to Office (RTO): Maximum 5 days off with no more than 2 consecutive; no RTO on holidays, weekends, or the day before/after holidays

Required Qualifications

  • Valid LPN / LVN license in Illinois
  • Minimum 1 year of Long Term Care experience within the last year
  • Experience in a Corrections setting
  • Medical-Surgical nursing experience
  • BLS certification issued by the American Heart Association

Certifications & Competencies

  • Medication Test/Pharmacology Competency Exam (required for starts on/after 4/14/24, must be completed within 12 months of start)
  • Color Blind Testing (required for starts on/after 1/16/24, must be completed within 12 months of start)
  • Competency Exam (required for starts on/after 4/14/24, must be completed within 12 months of start)

Health & Safety Requirements

  • Physical exam within 12 months of start, updated annually (signed by PA, NP, DO, or MD)
  • Fit mask test within 1 year of start (N95 or equivalent), required annually; medical or religious exemptions require documentation and facility approval for onsite PAPR use
  • Tdap vaccine within 10 years or signed declination within 12 months of start
  • Negative TB test (QGold or TSPOT) within 12 months of start, updated annually; chest x-ray and TB questionnaire if history of positive TB
  • Immunizations:
    • Measles, Mumps, Rubella (2 MMR vaccines or positive titer; booster if low titer)
    • Varicella (2 vaccines or positive titer; booster if low titer)
    • Hepatitis B series, titer, or signed declination within 1 year of start
  • Seasonal flu vaccination or approved medical/religious exemption required during flu season (Oct 1 – May 4)
  • Negative 9-panel drug screen within 90 days of start, updated annually

Background & Documentation Requirements

  • Background check within 12 months covering all locations lived/worked/schooled in past 7 years, updated annually; includes SSN trace, nationwide, sex offender, and other relevant searches
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
  • Illinois Healthcare Worker Registry verification (not required for LPNs but required for aides and nursing assistants)
  • Government-issued photo ID (driver’s license or passport)
  • Completion of facility-specific forms with legal name and signatures
  • Non-employee application with PREA clearance (time sensitive, must be completed within 24 hours of contract signing)
