Location: Wenatchee, Washington
Assignment Length: 16 weeks
Shift: Day shift, 5 days a week, 8 hours per day (9:00 AM - 5:00 PM)
Unit: Skilled Nursing Facility (SNF)
Job Summary
Seeking a Physical Therapist Assistant (PTA) for a travel contract in a Skilled Nursing Facility setting. The facility is located near the Columbia River, approximately 24 miles from Leavenworth, WA.
Required Qualifications
- Valid Physical Therapy Assistant license in Washington
- Minimum 1 year of Physical Therapy Assistant experience within the last year
Required Documentation & Compliance
- Government-issued photo ID (Driver’s License or State ID).
- Emergency Contact Form (required prior to start).
- Background checks within 1 year of start, including SS Trace, County searches (7 years), Nationwide, and Sex Offender registry. Annual updates required for EPLS/GSA/SAM and HHS/OIG checks.
- Physical exam within 1 year of start.
- Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, a negative chest X-ray within 1 year is required. Annual TB questionnaire also required.
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine series, or history of chickenpox)
- 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable).
- Seasonal flu vaccination or declination required during flu season (October 1 - March 31).
- Professional references:
- 2 references for travel, allied, or per diem acute care roles.
- 1 reference for per diem non-acute care roles.
- Annual skills checklist and competency exams as applicable.
- Medication test/pharmacology competency exam required for medication aides and RNs (if applicable).
