Location: Yuma, Arizona
Contract Length: 13 weeks

Provide physical therapy services in a home health setting with home visits and periodic office meetings.
Average patient load: 5-6 patients per day, approximately 28-30 visits per week.
Typical work schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have alternate schedules such as 4x10 shifts (Friday-Monday).
Must be willing to travel up to 75 miles from the home clinic, averaging 150-200 miles traveled per week. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
Use of Homecare Homebase (HCHB) electronic medical record system.
Types of cases: Start of Care and Follow-Ups.
