Schedule
- Generally 5 days per week, with likely one weekend day
- Variable day shift
- May require floating to other units or facilities based on census and staffing needs
- Must be available for Hawaii State and Federal Holidays as needed
Required Qualifications
- Valid Physical Therapy Assistant license in Hawaii
- Physical Therapy Assistant with 2 years of experience
- Physical Therapy Assistant experience within the last year
- BLS
Responsibilities
- Provide rehabilitation services in a Skilled Nursing Facility and/or Intermediate Care Facility
- Perform evaluations, reassessments, treatments, and other clinical services within scope of practice
- Develop, implement, or carry out individualized treatment plans with the supervising therapist, physician, and interdisciplinary team as appropriate
- Deliver evidence-based therapeutic interventions to support function, independence, communication, mobility, swallowing, and quality of life as applicable
- Educate residents, families, and caregivers on treatment plans and patient progress
- Complete accurate, timely, and compliant documentation
- Document progress, plan-of-care changes, and discharges according to applicable requirements
- Supervise therapy assistants when applicable
Training / Orientation
- Orientation to the rehabilitation department, documentation system, and facility workflows is provided
