Schedule

• Generally 5 days per week, with likely one weekend day
• Variable day shift
• May require floating to other units or facilities based on census and staffing needs
• Must be available for Hawaii State and Federal Holidays as needed

Required Qualifications

• Valid Physical Therapy Assistant license in Hawaii
• Physical Therapy Assistant with 2 years of experience
• Physical Therapy Assistant experience within the last year
• BLS

Responsibilities

• Provide rehabilitation services in a Skilled Nursing Facility and/or Intermediate Care Facility
• Perform evaluations, reassessments, treatments, and other clinical services within scope of practice
• Develop, implement, or carry out individualized treatment plans with the supervising therapist, physician, and interdisciplinary team as appropriate
• Deliver evidence-based therapeutic interventions to support function, independence, communication, mobility, swallowing, and quality of life as applicable
• Educate residents, families, and caregivers on treatment plans and patient progress
• Complete accurate, timely, and compliant documentation
• Document progress, plan-of-care changes, and discharges according to applicable requirements
• Supervise therapy assistants when applicable

Training / Orientation

• Orientation to the rehabilitation department, documentation system, and facility workflows is provided
