Job Description
Join a dedicated home health team providing compassionate, one-on-one, in-home care focused on promoting strength, mobility, and independence. This role offers the opportunity to make a lasting impact through personalized therapy plans and collaborative care.
Key Responsibilities
- Evaluate patients and develop personalized therapy plans in the home setting
- Deliver evidence-based interventions to restore mobility and function
- Educate patients and families on exercises and safety strategies
- Collaborate with interdisciplinary teams for coordinated, goal-oriented care
- Maintain accurate and timely documentation to meet quality and compliance standards
- Adapt care plans based on patient progress and physician feedback
- Support patients through discharge planning and wellness strategies
Required Qualifications
- Licensed Physical Therapist (PT) in the state of New Mexico
- Minimum 1 year of clinical experience (home health experience preferred)
- Skilled in patient communication, documentation, and problem-solving
- Self-motivated and comfortable working independently
- Proficient with electronic health records and mobile charting
- Passionate about function-focused, relationship-driven care
