Job Description

• Provide direct care and supervise care given by CNAs, LVNs, RNs, and support staff in a long-term skilled nursing setting.
• Ensure continuity of care by communicating with physicians and healthcare team regarding changes in patients' clinical conditions, diagnostic results, and symptomology.
• Respond quickly and accurately to changes in patient condition or treatment response.
• Participate in Quality Improvement activities.
• Assess, plan, implement, and evaluate nursing care of residents within the skilled nursing facility.

Required Qualifications

• Current California RN license.
• Minimum 2 years of experience in a long-term care facility.
• Working knowledge of Title 22, OBRA, and other relevant regulations.
• Operating knowledge of Electronic Medical Records preferred.
• Current BLS certification.
• Ability to effectively communicate in English (verbal and written).
• Good interpersonal skills; able to interact professionally and respectfully with residents, patients, staff, and external agencies.
• Basic computer skills.
