Physical Therapist - Contract Opportunity

Location: St Albans, Vermont
Employment Type: Contract
Contract Length: 13 weeks


Job Description

  • Actively manage and adjust patient visit schedules to maximize productivity and meet patient needs.
  • Perform comprehensive patient assessments including psychosocial, educational, safety, and risk factors in compliance with federal/state regulations and professional guidelines.
  • Develop individualized plans of care to promote functional independence and safety, setting patient-specific goals and logical interventions.
  • Provide skilled physical therapy treatments, including therapeutic techniques and patient/caregiver education, while assessing patient progress at each visit.
  • Coordinate patient care with interdisciplinary team members to ensure optimal outcomes and communication.
  • Supervise and direct clinical activities of Physical Therapist Assistants (PTAs), PT students, and home care aides.
  • Complete and coordinate required documentation and paperwork to reflect care provided, support billing processes, and comply with regulations and standards.
  • Participate in in-service training programs for staff and community groups.
  • Maintain and enhance professional knowledge and promote physical therapy standards.
  • Engage in marketing and promotion of rehabilitation services, including community outreach.
  • Perform various patient health assessments (e.g., PT/INR, blood pressure) and procedures (e.g., staple/suture removal, wound care).
  • Communicate changes in patient care schedules promptly to schedulers and supervisors.
  • Complete OASIS documentation and collaborate with Quality Improvement staff to ensure accuracy.
  • Follow agency protocols for visit documentation and billing processes.
  • Assess patient needs for home care aide services, develop and coordinate plans of care, and provide supervision as required.
  • Coordinate safe patient discharges and transfers, ensuring all paperwork and information comply with policies and regulations.
  • Report patient incidents (infections, injuries) timely to Quality Improvement specialists.
  • Participate in regular peer review audits, providing and accepting professional feedback.

Required Qualifications

  • Valid Physical Therapist license in Vermont
  • Minimum of 2 years of experience as a Physical Therapist
  • Recent Physical Therapist experience within the last year
  • Experience in supervisory roles
