Location: Lebanon, New Hampshire (with service areas in surrounding VT and NH towns)
Employment Type: Contract
Contract Length: 16 weeks

As an RN Case Manager in Home Health and Hospice, you will:

Screen, evaluate, re-evaluate, and treat patients in their homes.
Coordinate and communicate treatment activities with an interdisciplinary care team.
Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
Provide ongoing assessment of patients’ clinical, functional, and emotional status.
Communicate significant changes to physicians and team members.
Document evaluations, treatment goals, and objectives.
Teach patients and caregivers self-care techniques, medication, diet, and provide health counseling as appropriate.

This role offers broad independence while being part of a cohesive patient-care team, with a focus on work/life balance and flexibility.

Valid RN license in New Hampshire
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Minimum 2 years of Home Health experience, including experience within the last year
Experience in Hospice care
Current ACLS certification
Current BLS certification
