Job Opportunity: Registered Nurse - Care Coordination/Case Management

Location: Carmichael, California
Contract Length: 13 weeks (approximately 3 months)

Job Summary

Seeking an experienced Care Coordinator for a travel contract position. The role involves discharge planning and utilization management across all hospital areas (no outpatient or home care). Candidates will work primarily 8 am to 5 pm shifts with at least one full weekend per month.

Key Responsibilities

• Manage 15-20 patient consults per day
• Perform initial assessments, discharge planning, and utilization review
• Work with all payers, including uninsured and homeless populations
• Use Interqual criteria and Cerner EMR (training provided)
• Collaborate effectively with hospital staff and reprioritize tasks as needed
• Support both discharge planning and utilization management functions

Required Qualifications

• Active California RN license
• BLS certification
• Minimum 2 years of hospital experience
• Minimum 2 years of experience in care coordination/case management
• Experience with discharge planning and utilization management required
• Prior recent experience working in California Dignity facilities is mandatory
• Ability to work every other weekend (at least one full weekend per month)
• Strong communication, teamwork, and collaboration skills

Additional Information

• Orientation includes 5 days of care coordination training plus unit-specific orientation
• Business casual attire required
• Hospitalist and Respiratory Therapy available 24/7; Pharmacy not 24/7
• No holiday requirement, but willingness to work holidays is appreciated

Notes

• Candidates must not have been employed full-time, part-time, or PRN by certain affiliated health systems within the past year (exceptions for PRN through outside firms)
• Two supervisory references required at submission (one within 12 months, one within 3 years)
