Contract Length: 13 weeks

Hours: 40 hours per week guaranteed
Weekend/Holiday: One weekend per month required
On-call: Not required, but may be called during low census hours with callback pay
Patient Load: 20-25 patients total, averaging 4.5 visits per day
Travel: Daily travel varies from 20 to 100 miles depending on territory and patient census
Charting System: Epic preferred
Patient Ratio: 4-5 patients per day

Experience: Minimum 2+ years Hospice nursing experience
Certifications:
- BLS
- Active Wisconsin or Compact nursing license
- Valid Driver’s License
- AHA or ARC certifications required by employer

Scrub attire: Any scrubs or business casual allowed
Contract includes eligibility for 36 hours of low census unbillable hours with potential callback pay
Guaranteed hours per week except during holidays when the unit is closed
