• Employment Type: Travel/Contract
• Average Patients per Day: 5-7 (please confirm during interview)

• Territory: Up to a 60-mile radius from the hub/office in Suffolk, VA

Required Qualifications

• Valid Occupational Therapist license in Virginia
• Minimum 1 year of Occupational Therapist experience within the last year
• Experience working with Geriatric patients
• Experience in a Home Health setting
• Experience with Homecare Homebase (HCHB) EMR
• BLS (Basic Life Support) certification

Required Compliance & Documentation

• Background checks including SSN trace, county searches, FACIS, nationwide sex offender, motor vehicle records, education and work history verification, and state-specific searches as applicable
• EPLS/GSA/SAM and HHS/OIG checks within 90 days of start
• Fingerprinting as required by state regulations
• Education verification of highest degree related to profession
• Medication/Pharmacology competency exam as applicable
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Proof of current auto insurance (required for all Home Health positions involving driving)
• Facility-specific policies including Acceptable Use Policy and Flu/COVID vaccination or declination documentation
• Physical exam within last 12 months
• Negative TB test or chest X-ray as applicable
• Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
• 9-panel drug screen within last 12 months
• References: 2 professional references within 2 years of start
• Employment application with full 7 years of work history, no gaps
• Work history verification for 7 years

• Timely documentation and communication are required
• No cell phone use allowed during work
• Must comply with dress code and facility policies

Prior Travel Experience

• Not explicitly required; first-time travelers accepted
