Required Qualifications

• License: Must hold a current, active New York State license for Radiology and ARRT certification.

  • Physical exam within 12 months prior to start, including vital signs, medical history, and clearance to work without restrictions.
  • Fit Mask test (N95) within 1 year prior to start.
  • Negative TB test or chest X-ray as per options provided.
  • Immunizations/vaccine records for Rubeola (Measles), Mumps, Rubella, Varicella, Hepatitis B, Hepatitis C, and seasonal Flu vaccination (required during flu season).
  • Drug screening (9-panel) with chain of custody within 60 days prior to start.
  • Eye exam (vision screening) within 12 months prior to start.
• Competencies:

  • Medication Test/Pharmacology Competency Exam as applicable.
  • Color Blind Testing (online or in-person if required).
  • Competency Exam relevant to specialty.
• Additional Requirements:

  • Signed Orientation Safety Manual Acknowledgement.
  • Completion of facility-specific orientation and training modules prior to or at start.
  • Compliance with New York State Central Registry and Statewide Exclusion List forms and results.
  • Employment application and work history verification covering 20 years.
  • Professional references: 2 for travel/allied/per diem acute care roles.
