Registered Nurse - Skilled Nursing Facility

Job Description

• Provide nursing care in a skilled nursing facility with regulations similar to acute care hospitals.
• Patient population includes many short-term rehab patients (fractures, strokes, long-term IV antibiotics).
• Average patient length of stay: 2-3 weeks.
• Common diagnoses: Osteomyelitis, hip and knee replacement or fracture.
• Facility has 8 rooms/16 beds, average daily census of 10.
• Nurse-to-patient ratio typically 1:5, maximum 1:7 (rare).
• Work schedule includes every other weekend.
• Float within areas of competency (MedSurg 4100 and 5100).
• No on-call duties.
• Use Omnicell medication system and Meditech Expanse documentation.
• Communication via SBAR.
• Dress code: Navy blue scrubs (not provided, no logos or patterns).
• First-time travelers are welcome.

Required Experience

• Minimum 2 years of nursing experience.

Required Certifications

• BLS (Basic Life Support)
• MAB (Management of Assaultive Behavior)

Required Skills and Competencies

• Blood glucose monitoring
• NG tube insertion and management
• Ostomy care
• Tube feedings (Gravity, Syringe, Pump)
• Continuous Bladder Irrigation (CBI)
• Foley catheter insertion (female and male)
• Nephrostomy tube care
• Peritoneal dialysis/CAPD
• Central line and peripheral line care/management
• PICC line management
• Starting and maintaining IVs
• Anticoagulation therapy
• Antihypertensives
• Reversal agents
• Care for patients with CVA (stroke), neuromuscular disease, spinal cord injury, traumatic brain injury
• Use of Hoyer lift
• Tracheostomy care and management
• COVID patient care
• Suctioning (ETT, NT)
• Pneumonia care
• Complex wound care
• Infection prevention and isolation precautions/PPE

Additional Requirements

• Must comply with facility policies including signing various acknowledgements related to abuse reporting, confidentiality, code of conduct, and safety.
• Required to complete background checks and verifications within 30 days of start.
• Must provide required health documents including physical exam, immunizations, TB screening, drug testing, and fit mask testing.
• COVID vaccination plus 1 booster required or approved exemption.
• Must pass medication and competency exams as applicable.
• Must adhere to facility dress code and photo guidelines for badge.

Summary

This is a travel nursing contract for a Registered Nurse with at least 2 years of experience, working in a skilled nursing facility setting with a focus on short-term rehab patients. The role requires a broad set of clinical skills, certifications in BLS and MAB, and compliance with extensive health and facility-specific documentation and policies. Prior travel experience is not required.
